Please complete the form below. Required fields are marked with an asterisk*. To complete the address change on your child(ren)'s school record, you must attach a copy of your new lease, mortgage, or tax statement containing your name and new address as well as a copy of one of the following: a utility bill, satellite TV/cable bill, or landline phone bill containing your name and new address. If you are living in a relative's home, please read here for further instructions. (Please know that you might also be required to show your original documents to the school secretary once school is back in session in the buildings.)
If you do not wish to complete this application online, please contact your child's school or the district office to receive and return the forms by mail.
